The Arm-Twister is a faerie and supporting character in Dragon Quest I HD-2D Remake. She is the one who draws the Hero into midst of the fair folk and plays an instrumental role in helping save Alefgard from the Dragonlord.

The Arm-Twister is first encountered in the depths of the Craggy Cave, along with her best friend the Day-Dreamer. The two were exploring the dungeon in the hopes of finding the Dream-Whisperer, who recently disappeared from the near by Faerie Village, but they were cornered by a monster before being inadvertently saved by the Hero. Their whispering led the Hero to them, causing the Day-Dreamer to panic while the Arm-Twister stays calm; she is willing to take a chance on the Hero as he slew the monster after them. She asks him his name and receives the hero's mission up to that point, and is surprised that he knows of Rubiss. The Day-Dreamer chides her for attempting to reveal fair folk secrets to a smelly human, but the Arm-Twister vouches for him and points out that the Dragonlord taking over wouldn't be good for their kind either. The two whisper about manipulating him into helping achieve their own ends, and implore the Hero to visit the Cave of the Dwarves in the east to find a lead on the missing Dream-Whisperer.

Later, at the Faerie Village, the Over-Seer tries to frighten the Hero off with a booming voice, but the Arm-Twister vouches for him. The Hero explains that the dwarves had no leads to the Dream-Whisperer's whereabouts and that the one whose help he needs is sick with a swamp fever normal medicine cannot cure. The Over-Seer sadly tells him that they do not know of the two crucial ingredients to make the medicine―sunup syrup―and allows him to spend the night in the village. The Hero is then contacted by the Dream-Whisperer who informs him of the missing reagents. When the Hero returns with the ingredients necessary for concocting the special cure-all he finds the Arm-Twister, Day-Dreamer, & Over-Seer under attack from the Dragonlord's forces. After slaying the beasts and handing over the ingredients, the Arm-Twister and Day-Dreamer brew a phial of sunup syrup for him.

The Arm-Twister is then seen creating the soul sigil along with the other faeries. When the Hero speaks to the Over-Seer about the sigils and their purpose in summoning Rubiss, the chief of the faeries demands to know who blabbed about their secrets to a human. The Day-Dreamer promptly rats the Arm-Twister out, causing a girly scuffle. The Arm-Twister defends her loose lips by saying it all worked out in the end. The Arm-Twister remains in the faerie Village for the remainder of the game, until it is attacked by a larger force of monsters that suddenly materialize within it. When a dragon zombie corners her and the Day-Dreamer, she pushes her friend to safety and takes the mortal blow. Her final words were urging the Day-Dreamer to flee, assuring her she'll be right behind her.
