It can be purchased in shops for 320 gold coins in the NES version, but in remake versions of the game, it is obtained by turning in 5 mini medals to the medal collector in Aliahan. It can also be sold for 240 gold coins. Regardless of versions, Overtoads can sometimes drop them.
