The Quadrahead is a monster that first appeared in Dragon Quest V: Hand of the Heavenly Bride.

As its name implies - it is a collection of four demonic heads. Three of the demon heads are tiny, round, and one eyed. The one at the top is a two horned monster possessing long, wavy orange hair. The other two in the center left and right respectively possess a single horn each and a pair of pointy ears. These three one-eyed demons ride atop a larger, humanoid shaped demon who has a wide face with two darkly evil eyes, pointy ears and a mouth holding some very sharp fangs.
