Special antidote is an advanced healing item in the series. It cures a single party member of the effects of poison and restores a moderate amount of HP. In games where Alchemy appears, it can typically be created by combining two strong antidotes or other less potent herbs together.

Special antidote can restore 60~80 HP to a single character and cures the effects of poison and envenomation. In addition to being made in the Alchemy Pot, some can be found at Marta's Cottage (until receiving the Ultimate key) and Dark Empycchu. In the original Sony PlayStation 2 version, special antidote can be sold for 275 gold each for the first five sold, reducing to 193 gold each for the sixth through tenth, then dropping even further to only 83 gold each from the eleventh sold onward.
- In the Nintendo 3DS version, a special antidote can be found in a pot at the Altar of Wroth. It can also be found randomly in blue treasure chests in the Kingdom of Trodain, the island where Princess Minnie's Castle is located, the Savella Region, and the Holy Isle of Neos.

Dragon Quest IX: Sentinels of the Starry Skies[edit]

Special antidote still cures the effects of poison and envenomation, while the amount of HP restored has increased to 90~120. There is now only one recipe to make one in the Krak Pot and its sell price has reduced to 33 gold. It can also be used to create a Contra band.
Dragon Quest X[edit]
Special antidote now only restores around 60~70 HP to a single character, in addition to curing poison and envenomation. In Version 1.0 of the game, it could only be acquired from certain sparkly spots or dropped by monsters. In the game's Offline mode, it can be created once again by combining two strong antidotes together. Starting from Version 5.0, it can be dropped rarely by Vicious bubble slimes.
- In the Offline version of the game, special antidote can be made by combining three strong antidotes together.
Dragon Quest XI: Echoes of an Elusive Age[edit]

Special antidote can be purchased for 87 gold from various item shops and roving emporiums starting from the Hotto Steppe during the second act and most shops in the postgame and sold for 43 gold. Some doses of special antidote can be found at the Last Bastion and Dundrasil Castle during the second act. It can also be dropped by Vicious toxic toads, Vicious bodkin fletchers, Merkings, Gnashturtiums, and Fromage greys.
Special antidote can be used in a single recipe for the Fun-Size Forge:
- Recipe: Ring of immunity = Snakeskin x2 + Special antidote x3 + Coagulant x3
Dragon Quest of the Stars[edit]
Special antidote cures poison and restores around 60 HP to a single party member. It can be created at the item workshop by mixing two strong medicines and two antidotal herbs together. It can also be received as a reward for completing certain multiplayer levels.
